Chronic Suppurative Otitis Media (CSOM) Ayurvedic Treatment

The perforations (minute holes) in the tympanic membrane/ ear drum due to persistent inflammation of middle ear, resulting in continuous discharge from ear is called chronic suppurative otitis media/ CSOM. It is actually a complication of acute otitis media (inflammation of middle ear). It is caused by infection of middle ear and mastoid cavity.

Signs and Symptoms

Recurrent/ persistent discharge from ear lasting for 2-6 weeks, holes in tympanic membrane, thickened middle ear mucosa with polyps, fever etc are the common signs and symptoms of CSOM. Gradually, the person develops hearing problem.

Ayurvedic Analysis

Karnasrava (oozing from ear) is a disease of ear mentioned in Ayurveda. The main feature is discharge of pus from ear.

Causative Factors (Ayurveda)

Krimi (infection) followed by pooya (suppuration) is the main cause of karnasrava. Injury or disease of head, excessive exposure to polluted water (swimming, games etc), and abscess in ears are mentioned as the reasons of ear infections. Vata is the vitiated dosha here.

Possible Complications

The most common complications are infection of bones in the inner ear and other structures like labyrinth which may affect brain. Other major complications are hearing loss, problems with speaking and behaviour.

Other systemic diseases and precautions to be taken

Consider other infections, hypertension etc before planning the treatment.

It can be managed by internal medications along with pramarjana (cleaning the ear), karnadhupana (fumigation of ear cavity with medicated fumes), karnapoorana (retention of medicines in ear), nasya (nasal instillation of medicine) etc.

Include more whole grains, vegetables, pulses, nuts, milk, fruit/ fruit juices in diet. Avoid spicy/ cold/ dry food, bitter/ astringent/ pungent tastes etc.

Avoid exertion during discharge from ears. Take proper rest.

Preventive Aspects

Maintain ear hygiene. Avoid contact with polluted water resources. Take proper care of infectious diseases. Avoid speaking loudly/ for long periods, exposure to cold and dry air.

Importance of Diet

Reduce the intake of food that increases vata, as they can worsen the infection and discharge. Follow the diet pattern directed by physician during treatment.

Take proper care of ears. Avoid chances of infection. Avoid heavy exercises, travelling a lot, swimming etc.
